The Northeast Health Plan- Part B is a community sponsored health benefit program that serves uninsured adults in Alcona, Iosco, Ogemaw, and Oscoda counties.

Who is Eligible?
• Adults aged 19-65 (ages 19-21 must be denied at DHS before becoming eligible for Northeast Health Plan)
• Have no health coverage
• Income qualification
• Live in Alcona, Iosco, Ogemaw, or Oscoda County
• U.S. Citizen
Member Services and Co-Payments
• $5 per Physicians Visit
• $10 per Specialist Visit
• $20 per Urgent Care Visit (emergency room NOT covered)
• $5 generic prescriptions/ $10 brand prescriptions (some prescriptions not covered)
• $5 for Radiologist (x-rays)
• Lab work also covered
• No coverage for hospital services

*If you state that you have no income, you must apply for benefits at your local DHS office. If you are denied benefits, you can become eligible for the Northeast Health plan.
